Disquiet Ending Explained: Does Sam Manage to Get Out of the Hospital?

Waking up to a reality more terrifying than dreams.

A recently released horror film on Netflix features Jonathan Rhys Meyers, well-known for his role as Henry VIII. Disquiet is the latest psychological thriller that keeps viewers on the edge of their seats.

Meyers plays Sam, a wealthy and confident tech entrepreneur who suffers severe injuries in a car crash after a reckless truck driver slams into his vehicle.

A Strange Awakening in an Empty Hospital

In a fashion somewhat similar to 28 Days Later, Sam regains consciousness in what appears to be an almost deserted hospital. The last memory he has is of his wife telling him she loves him. As he wakes, he finds himself in a hospital bed beside a patient who seems to be in a coma.

However, the reality is far from that—the individual is fully conscious, displaying an unnatural level of speed and agility. Sam is suddenly attacked and has no choice but to defend himself using a scalpel.

Before things escalate further, a mysterious nurse intervenes, sedates the aggressive patient, and quietly takes him away without explanation.

Unusual Happenings Within the Hospital

Strange occurrences are everywhere in the facility. A woman named Monica, who checked in for breast enhancement surgery, is among those experiencing disturbing events. While lying on the operating table, awaiting anesthesia, she notices something horrifying—the surgeons around her have no faces.

Although she initially survives the procedure, her nightmare is far from over. Upon waking, she is confronted by three blood-soaked women, their bodies wrapped in bandages. Sam hears Monica’s terrified screams as she is attacked, and he rushes to help.

After managing to fend off one of the attackers, he and Monica escape. They soon encounter a Black man named Carter, who was admitted to the hospital after being shot by a racist police officer.

A Twisted Connection Between Victims and Perpetrators

Things become more complicated when it is revealed that the very police officer who shot Carter is also in the hospital, recovering from injuries sustained in the same incident. Sam, Monica, and Carter try to find a way out and head towards a fire exit.

However, just as they think they have found their escape route, Sam notices something disturbing—flames are slowly consuming the building from below, blocking their intended path. Before they can react, an orderly suddenly appears and knocks Sam unconscious.

Surprisingly, rather than causing harm, the orderly later tends to Sam’s wounds before he regains consciousness.

The Search for a Way Out

Shortly after, another figure enters the picture—a wheelchair-bound man named Virgil. He suggests to Sam that their best chance of escape is through the hospital’s rooftop, as all means of communication within the building have been cut off.

Meanwhile, the doctor who had earlier treated Sam returns and proposes a different escape plan. He insists that the ground floor is the safest route to leave the hospital. Oddly enough, both Virgil and the doctor seem to possess an unsettling amount of knowledge about Sam’s past.

What Happens at the End of Disquiet?

As Sam gazes outside, he is met with absolute darkness. He begins to suspect that a large-scale terrorist attack may have taken place in the United States. However, the truth behind the horrors of Disquiet is far beyond anything rational.

The hospital itself is not what it seems. It is soon revealed that Sam, along with the others in the facility, is trapped in a state of limbo—a space that exists between life and death. Sam and the other characters had already lost their lives in the real world and were now stuck in the hospital, unable to move on to the afterlife.

This explains why every attempt to escape the building had been futile. Those who either die inside the hospital or choose to stay eventually transform into the terrifying entities that have been attacking them.

A Battle Between Two Forces

Virgil and the doctor take on symbolic roles, representing opposing forces—Virgil works to guide souls toward heaven, while the doctor is determined to drag them into eternal suffering. As Sam and Monica continue their desperate search for an exit, they arrive at the morgue.

There, they make a terrifying discovery—the bodies in the room begin to move. These reanimated corpses are, in fact, the very people responsible for the tragic circumstances that led each character to the hospital in the first place.

One by one, everyone except Sam is seized by an unseen force, with the doctor seemingly in control of everything. Just as it seems like there is no hope, Virgil steps in and rescues Sam.

The Final Test and Sam’s Awakening

Virgil and Sam proceed to the maternity ward, where Sam is confronted by an apparition of his wife. She urges him to return to his family. During this emotional exchange, Sam finally understands the truth—he is trapped in limbo, and the old man he encountered earlier was not a stranger, but rather an aged, deranged version of himself.

Realizing that the only way to escape is through the rooftop, Sam fights the temptation to take the basement route. With sheer determination, he reaches the roof, and in that moment, he wakes up in the real world, alive once again.

His first words upon waking? He tells his wife that he loves her—mirroring the last thing he remembered before the accident.
